This position will require the LPN to travel to our patients' home to complete supervisory visits with our clients receiving personal care assistance.Responsibilities

• Conduct Initial and Annual Skills Training of PCAs.
• Re-evaluation of patients' skilled nursing/PSS needs
• Work autonomously with minimal assistance by the Clinical Manager
• Maintain patient confidentiality
• Make decisions based on established policies and procedures
• Preparation of the clinical progress notes.
• Informing the family and Registered Nurse of changes in the patients' condition or needs.
• Supervisory Visits and teaching in Patient's Home.
• Administration of medications and treatments as prescribed by a physician in accordance with currently accepted standards of medical practice
• Total patient care
• Patient and family teaching.
• Other services in accordance with and as outlined in the Georgia Licensed Professional Nurse Practice Act.
• Reports directly to the Registered Nurse and Clinical Director.Qualifications
  • Licensed Practical Nurse (LPN)) must hold a current unencumbered Nursing license to practice in the state of Georgia.
  • LPN will complete the assigned onboarding nursing quiz.
  • LPN should have experience caring for medically fragile individuals.
  • It is preferred that the LPN have experience in the home health service field.
  • Current BLS Certification.
