Physical Therapist Location: Griffin, GA Agency: Cynet Health Pay: $2,446 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 3/16/2026 About the Position Job Title: Physical Therapist (PT) Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health Experience License: Must have a valid license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience is required. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Description: This position involves providing home health therapy. The clinician will conduct home visits and attend periodic office meetings. Average number of patients per day is 5-6. Contractors will be assigned between 5-10 patients per day. Each visit and activity including documentation and travel holds a different point value. Clinicians are expected to reach productivity points of 30 per week. There are various types of clinical cases including start of care and follow-ups. Weekend coverage requirements vary per location and profession. On-call duties may apply once a month. Clinician must be willing to travel up to 75 miles from their home base. Mileage will be reimbursed at the IRS standard rate. All documentation must be completed within 24 hours of the patient appointment. Declining patients may result in cancellation of the contract. This assignment does not guarantee hours. The clinician must accept every patient on their schedule. Delayed or incomplete documentation that impacts billing and follow-up care will lead to cancellation.
